If you choose to mail it in please send it to the address on the order form. For your convenience we take Discover, Visa, and MasterCard or you may choose to pay by check.

IMPORTANT:
You must type in the school code with all lowercase letters, no spaces and no special characters.
Example: Your School Code should be typed yourschoolcode in the box below. If your school code has a prefix like St., type styourschoolcode in the box below.
